Long Term Personal Care Plans
Our Sonas care teams are highly skilled and work closely with residents and their families to create a personal care plan for each resident, designed to meet their wishes, wants, as well as their needs.
The agreed detailed care plan is devised to suit the residents’ individual needs and detailed records are maintained and updated regularly. This ensures continuity and allows Sonas to continually assess the plan for improvements or changes based on outcomes and feedback.
The Sonas care teams provide residents with high levels of privacy, comfort, and expert care. Our knowledge and experience come from staff members who have been there since our inception and newer members of the Sonas care team family are inducted by older members of staff, ensuring they embrace and live by their core values.
Services & Activities
As well as on-site registered nursing care, residents can avail of a wide range of services as part of their care plans, including;
- Occupational therapy
As well as a range of beneficial services, we provide special dietary needs catered for by a qualified chef. Our Activities Teams provide a full programme including day trips and outings, games and activities to take part in, computer classes and exercise classes. All our activities are designed to encourage participation in daily activities that promote physical, mental and social wellbeing.
If additional services are required we can organise to have that service brought to the resident or alternatively to a local facility where that service is provided.
