Social Care Practitioner

Candidates who have completed Fetac Level 7/8 or above Qualification in Social Care Work/ Social Studies will be considered.

Role Summary: To promote and maintain the highest quality of social support and care to the residents in the Nursing Home. To work with the clinical team to deliver assessment-based personalised care to each resident. Developing a care plan which reflects both the assessed needs of the resident and their personal wishes. To advocate for, and protect the rights of the resident. To supervise and delegate to care staff in the delivery of social and clinical care to residents. To document (electronically) and communicate all social care interventions in line with policy and regulation.

Duties & Responsibilities will include but not limited to:·

  • Deliver a high standard of person-centered care ensuring all practices reflect the privacy, dignity and respect of our residents and underpin all the basic values that contribute to an excellent quality of life.
  • Support and assist residents enabling them to fulfill their activities of daily living ensuring that residents are encouraged to be as independent as possible.
  • Liaise with the multidisciplinary team to ensure balanced approval to care.
  • Participate in the monitoring and recording of the progress of the residents in line with agreed procedures.
  • Have a good knowledge of the Health Act 2007 in relation to residents in a long-term care facility.
  • Demonstrate an awareness of the HIQA standards in relation to the care of older adults in a long-term care facility.
  • Maintain professional standards in line with the regulations set out in the Health and Social Care Professionals Act 2005.
  • Participate in the monitoring and recording of the progress of the residents in line with agreed procedures.
  • Maintain clear, concise and accurate records at all levels within the home.
  • Support the Person in Charge in providing a high-quality service.
  • Supervise Healthcare Assistants and delegate duties as necessary
  • Support and assist the management team during internal and external audits/inspections.

Education & Qualifications:

  • Level 7 or Level 8 (Degree) or above Qualification in Social Care Work/ Social Studies desirable
  • Candidates with a FETAC/QQI Major Award Level 5 in Social Care or related field may be considered.

Skills and Experience:

  • Minimum 1 year previous experience as a Social care worker within a residential care home setting.
  • Possess a high regard for and practice good health and safety procedures at all times.
  • Passionate about delivering outstanding care to older people and be a committed, patient and caring person.
